When I was using these, I found the easiest way to get them to stay was placing a soldering iron near the end.

It tended to 'mushroom' the end slightly, which meant it didn't slip back, and it gave a slightly wider viewing angle.

Plastic fibre tends to be 21 deg angle of light.

You can epoxy or Araldite.

Try not to scratch the outer coating and watch out for cracking the fibre.
