With heat, plastic is supposed to expand by thermal expansion more than metal, so the plastic cap should tighten the connector more. &lt;p&gt;I suspect that there is a minor flaw in the cap design which enabled the internal pressure to push the cap off.&lt;/p&gt;
&lt;p&gt;It might be a safety feature.&lt;/p&gt;
