I noticed that when you soldered the IC there was quite a lot of bubbling still going on when you removed the heat. This is because the process had heated the solder paste too fast and there were volatile components of the flux left, probably under the chip. They are often associated with unmelted or unflowed solder paste balls or voids. For prototypes you can often get away with it but it is better, if you can, to make sure the paste has enough time hot but below solder meting point to boil off all the volatile stuff.
