I particularly liked the idea of using a distance sensor to provide some guidance of when the person is at the optimum distance for temperature measurement. You could add a simpler audio transducer to produce feedback to the user, a bit like a car reversing sensor. As you point out there are lots of problems depending on skin temperature to indicate core temperature.
