I'm unsure how you can make this statement

Nearly in every case of the accidents involving self-driving vehicles, human error was that cause of the problem and not the autonomous vehicle

and then counter it with this

There are two principal crashing causes involving self-driving vehicles:
- The sensors are not detecting what is happening around the vehicle
The software is mishandling unexpected situations when the vehicle faces conditions that is not programmed to do

Any vehicle that is fit for purpose should be ready for all these situations.

I think a more appropriate line would be :-

Nearly in every case of the accidents involving self-driving vehicles, unexpected circumstances not catered for by human programming was the cause of the problem

The failure of sensors is simply bad or poor engineering and could affect anything that is manufactured.

The fact it wasn't correctly tested before release is another issue, and warrants a recall as per other vehicle defects.

Your article raises a concern that many around here have been saying with autonomous drones.

What happens when an unplanned event occurs needs to be considered in the design, and should be identified early during the design process.

We've seen too many instances where a product is released and then found to have issues.

Luckily the medical and pharmaceutical industry are regulated.
