Creating the standards are easy, getting people to use them involves a lot of effort.

I spent a lot of time trying to convince engineers to use company standards for work and all I can tell you is I got a lot of lip service and very little compliance.

Some of it was laziness, some of it was fear, and some of it was just hubris that they were better than any standard.

You have to start at the school level, good luck with that, and then enforce it in the workplace. If only they permitted public hanging?

The only realistic option is to use Independent Verification and Validation teams to verify the data through each stage of development. A new pair of eyes can usually spot these types of errors. Not always, but most of the time.
