When using fans, the requirement is to move enough air through the hot areas of the system. This can be done with one fan and some appropriate ductwork, and the fan can be at the inlet or the outlet. Two fans are sometimes used to reduce the noise because they can be run slower, or sometimes to avoid using fancy ductwork, or sometimes it is simply overkill to avoid the effort of doing a proper thermal design. Since PCs can have an arbitrary set of upgrade cards, and designing optimum ducts is tough for an unknown configuration, there is something to be said for an overkill design.
