zynq mini-itx cannot boot

we have a mini-itx and it worked for two weeks after that it cannot boot in any mode and leds are in strange state just d22 and d23 are on when we power on board you can see picture of it in http://tinypic.com/view.php?pic=5buzv9&s=8#.VD4ZUdYvDZs
how to fix it?

In the picture it appears that the smaller four pin power connector from the ATX power supply is not plugged into the Mini-ITX power connector. You should insert the 4-wire connector first, on the end nearest the board edge, followed by the larger connector. Push the larger connector down until the retainer clip snaps into place.

i'm suspecious that something might be wrong with the power module. What can I do to debug the power module and pinpoint the problem? I've already probed J9 and J8 connectors while the board is on (and of course the power module is connected) and only see +12V on J8's pin 10; there's 0 on all other pins.

It does sound as if the power module is the problem. Please try reseating the power module to see if that corrects the issue.

I will contact you directly to gather information for a possible replacement.
