Does the output need to be a square wave with 50% duty cycle?
If not, you could just add a pulse after every second pulse.
If you need a true square wave you could use mixers or phase-locked loops.
Need to know more of the waveform you need, like sine or square or triangle or exponential or whatever, the importance of phase accuracy and delay, and the lowest and highest frequencies you anticipate encountering. In the electromechanical era, this sort of thing was sometimes achieved with synchronous motors and (i.e. brushes) contactors! In the world of lasers a non-linear crystal might be used.
