We're a distributor of EAGLE to North America so I should be able to answer
this 
The current EAGLE 5 license will work for all sub-versions of EAGLE 5. It
started at 5.0 and we're now at 5.11--those were all free upgrades. I'm
not sure if CadSoft is planning on releasing a 5.12 version.
EAGLE 6 has been announced for some time around 2011 Q4. That will be a
paid upgrade. CadSoft hasn't officially announced the cost of this upgrade
or even the exact release date. As a comparison, the cost to upgrade from
EAGLE 4.1 to EAGLE 5 (they used a different version system back then) was
~25-50% of the original license cost. Only CadSoft can give the cost but
that should be a rough estimate.
