how / where do you setup the default VIA settings (drill size, diameter)?

You could do this in the eagle.scr in the BRD: section, for example:

CHANGE DRILL 0.6mm;
CHANGE DIAMETER 2.0mm;
CHANGE SHAPE ROUND;
