Ask your cirquit board manufacturer how he would like to produce it.
I would draw lines on a separate layer describing the movement of the
center of a mill. My producer can mill down to 2mm width.
Ask your manufacturer what mill sizes he can use, and do not forget to
write down the use of that layer.

I think the best way is to reserve a layer for a 2mm mill (or whatever
is the smallest your manufacturer can use) and maybe a another layer
with larger mill.
Then draw your milling in thoose layers in your library, and use line
width same as mil width.
Another approach is to draw the hole edge using very thin line, and tell
your manufacturer that it is the edge you have drawn. My manufacturer
can cut arbitrary board shape that way. Then you must only know the
smallest mill diameter he can use.
