How do you set one up?

I was wondering if the board of the Pi comes with the Linux OS already installed onto the device. Or do you have to buy the SD card and write the OS to the card with a memory card reader?

The raspberry pi doesn't come with a SD card. You will have to get one and write the OS on it or buy a preloaded one.

Ah, ok.

What about screen output? I guess I would need a VGA to HDMi if I am going to use it on my computer monitor, right?

No. But you might need an HDMI to VGA if your monitor is VGA.
If that is the case, you may be better off upgrading to a newer monitor as HDMI to VGA converters are pretty expensive.

RasPi has HDMI and composite video outputs. You can find out more at the RasPi wiki: http://elinux.org/RaspberryPiBoard and http://elinux.org/RPi_Screens. The Wikis are immune from DDoS because so few people RTFM :-)
