From a brief examination the setup looks reasonable, and there's decent confidence if you say you tested beforehand and it worked.
Your actual deployment may be vastly different in unexpected ways, but it's not apparent from the notes. For example, the power source may be different, or cable lengths may be significantly different than the test setup.
You need to do some basic voltmeter checks to confirm the correct voltages are present on the correct expected locations, maybe even do a continuity check with power disconnected, because connectors may have issues. Check if the sensor is inductive or magnetic sensitive, and is it being permanently stuck high or low due to other metal nearby. At slow revolutions you could probably test with an LED and resistor, temporarily detatching the sensor output wire from the counter, and wiring to the LED instead.
Similarly, you could check the counter and wiring in isolation by temporarily disconnecting the sensor and applying a pulse manually.
Best to buy a couple of sets of components so you can have a permanent testbed set up to compare against, since the parts are quite low cost.
Thx Shabaz for good pointers. When trouble shooting I took all my set up apart and use small 12vdc battery and rigged up just sensor and display directly to eliminate connections/cabling continuity issues and tried to move the sensor ver the magnet. No results. Some people suggest that load should be included in the sensor circuits. Did not try this yet as I have no idea what resistor is required. Good idea with the LED. This should tell me if counter is faulty or sensor. Will try it.

The documentation you have on the counter looks like it includes an example circuit for counting with NPN type sensors. You have only included the switch example in your PDF though.
