Also, there was a loose screw in the anti-static bag with the board. It is from the heat sink. This is the second board purchased that is dead on arrival. Is there a severe quality control problem?

I have not heard of this issue thus far. All boards are checks for functionality at the manufacturer. In regards to your first board, did you contact someone at Avnet for help or are you in communication with your local Avnet FAE?
Please allow me to try and help trouble shoot.
1. What SD Card are you using?
- We reccomend use of a MLC UHS-1 class 10 MicroSD Card.

2. Are you using the MaaXBoard or MaaXBoard Mini?
- My assumption based on your post is the MaaXBoard, however I'd just like to confirm to avoid any confusion
3. Do you see any LEDs light up when you power the board?
4. What are you using to power the board? A dedicated power supply? Or plugging it into your PC?
- We reccomend a 5V/3A supply such as https://www.avnet.com/shop/us/products/avnet-engineering-services/aes-acc-maax-pwrul-3074457345642357173/
-This is especially important if you are plugging in a display or camera into the board.
5. Are you pluggin anything else into the board? (such as a camera or display)?
6. What program are you using to flash your SD Card? I personally use balenaEtcher https://www.balena.io/etcher/
4. What image did you program onto your SD Card? Please respond with the name of the image you programmed into the board. Please note, the MaaXBoard and MaaXBoard Mini have their out of box examples located on the same page. Please confirm you choose the correct image for your board.

I had issues myself with MaaxBoard - the one with HDMI port.
If I have HDMI port connected when powering up, it will not boot.
So disconnect HDMI cord and wait few minutes, then connect power again.
This is a real issue... of couple boards I have... should be addressed by Avnet.
